First, I had my husband cut a 4'x8' piece of plywood down to a 4'x5'4" piece.
Then I measured where I wanted to put the buttons and drilled holes where they would be.
This is the backside of the plywood after the first layer of padding had been attached.
In a black marker I wrote "TOP" where the top of the headboard would be, and then circled the holes for the buttons so that they would be easier to find later.

Here is the plywood after all three layers have been attached, and also the tools that I used to make this project. For the second and third layer of padding I pulled it higher so that I could cover the top part of the board to add padding there. And since the bottom part of the headboard would be behind the bed there didn't need to be as much padding.
Then I ironed the fabric to get all of the creases out.
Here is a close-up of the fabric. I love all of the different colors in it!
And the finished product! This is before it was attached to the wall. I will take that picture later once I make the bed with the new bedding I haven't picked out yet! :)
Overall this was an easy project that only took me about 3 1/2 hours to complete, and cost under $50.
